The Safety Net Medical Home Initiative (SNMHI) was a landmark demonstration of the Patient-Centered Medical Home Model of Care led by Qualis Health and the MacColl Center for Health Care Innovation. It was sponsored by The Commonwealth Fund and supported by eight local foundations.
We supported 65 primary care safety net practices serving 500,000 patients, including:
The SNMHI developed and demonstrated a replicable, sustainable model for transforming primary care practices into patient-centered medical homes with benchmark performance in quality, efficiency and patient experience.
All of the practices implemented the key features of the Patient-Centered Medical Home Model, and nearly half implemented the 33 identified key changes to a substantial degree.
In addition, the SNMHI built regional capacity for primary care improvement by training local practice coaches, and helped improve reimbursement and coordination among community resources.
The SNMHI has been identified by the Agency for Healthcare Research and Quality as an exemplary model for practice facilitation.
