Qualis Health was selected by the American Institutes of Research to provide technical assistance for the Centers for Medicare & Medicaid Services’ Federally Qualified Health Center Advanced Primary Care Practice (APCP) Demonstration Project.
The goal of the APCP Demonstration Project was to improve care, promote health and reduce the cost of care provided to Medicare beneficiaries served by 500 participating federally-qualified health centers (FQHCs) and 15 state Primary Care Associations. Participating FQHCs were expected to achieve Level 3 2011 NCQA PCMH Recognition.
Of sites that received direct technical assistance from Qualis Health, 81% achieved Level 3 Recognition by the end of the project.
As the lead technical assistance partner, Qualis Health worked directly with more than 115 practices around the country, along with practice coaches from each state’s Primary Care Association. Our support of the health centers included:
